Empire Health Survey Drawing

Rules:

One entry per person by completing the Native American COVID-19 Alliance survey. You have approximately a 1 in 10 chance to win a prize. You must be eligible and have participated in the Empire Health Foundation sponsered COVID-19 Indigenous Needs Assessment Survey and reside within the Empire Health Foundation service area. Only one prize per person. The examples below may or may not be the actual prize that winners will be awarded. The awarded prize will have an approximate cash value of the prize tier. You must be 18 years of age to participate in the study and drawing. Winners will be selected at random.  The drawing will take place after data collection has been completed. Initial drawing winners will be notified after data collection has been completed, no later than January 30, 2022* March 5, 2022. A link to check if you were a winner will be sent to all drawing entrants. Winners will be asked to confirm the U.S. mailing address of their current residence before their prize can be mailed. Drawing winners must respond with their valid U.S. mailing address within three days of the drawing. If we do not hear from the winner with a valid U.S. mailing address within that timeframe, we will select the next person from the list. Winners must use the same name for delivery as they submitted for the survey. Winners will be asked to confirm their identity within a stated time period.  The Native American COVID-19 Alliance, University of Washington, Empire Health Foundation, and IWRI reserve the right to refuse prizes to entrants that are deemed fraudulent, illegitimate, or otherwise invalid subject to the discretion of these organizations.
